How It Works
The Braden Scale predicts pressure injury risk using six subscales: sensory perception, moisture, activity, mobility, nutrition, and friction/shear. Unlike most scales, lower scores indicate higher risk (total range 6-23). Scores 15-18 indicate mild risk, 13-14 moderate, 10-12 high, and 9 or below very high risk requiring aggressive prevention measures.
